The first time you login, your login id and password will be the first letter of your first name, the first three letters of your last name, and the last four digits of your Social Security number. For example, John Doe, Social Security number 123-45-6789, would translate to the login id jdoe6789.

In Network: Select a VSP doctor and make an appointment. You may access the list of VSP doctors at www.vsp.com or request a hard copy thereof by calling VSP at (800) 877-7195. After you pay the $25 per person copayment, the Plan pays for the remaining covered services and eyewear you receive, subject to the limitations and exceptions of the Plan.
Out-of-Network Provider: You also have the option of receiving vision care services from any licensed optometrist, ophthalmologist, or optician who is not a VSP doctor. If you choose this alternative, you must pay for the services in full and submit an itemized receipt for reimbursement. The reimbursement schedule is shown in a section that follows. The reimbursement you receive takes into consideration the $25 per person copayment that must be paid before benefits are payable.

Your legal plan entitles you to hire your own out-of-network attorney and be reimbursed up to set dollar limits for any covered services. You will be responsible for any attorney’s fees in excess of these reimbursement limits and for any non-covered services you hire the attorney to perform.
To utilize this option, contact them at 1-800-821-6400 and ask any representative to send you a fee reimbursement packet.
